All records concerning me held by DAS in any division, including the Chief Human Resources Office, Risk Management, Enterprise Goods and Services, and the Office of the Director. This includes records indexed under Robert White, Robert Samuel White, Robert S. White, Sam White, or my date of birth. An extensive search has been conducted and no records exist. 2. All records reflecting any behavioral threat assessment, threat management, workplace violence assessment, or safety planning process concerning me. This includes but is not limited to: workplace violence incident reports; reports made under Statewide HR Policy 50.010.02; threat assessment team or workplace violence prevention team records, agendas, minutes, notes, or referrals; completed or partial assessment instruments of any kind; designations of me as a subject of concern, person of concern, or equivalent; safety plans; no-contact or no-trespass directives; and any recommendation to contact or involve law enforcement. An extensive search has been conducted and no records exist. 3. All communications between DAS and OPRD concerning me, including any consultation, guidance, or technical assistance provided by DAS to OPRD regarding my dismissal, my exclusion, my correspondence, my public records requests, the archive at oprdvolunteerabuse.org, or the March 24, 2026 police visit. An extensive search has been conducted and no records exist. 4. All communications between DAS and Oregon State Police, the Oregon Department of Justice, the Office of the Governor, or any federal agency concerning me. All records reflecting the disclosure of my personal information, including my date of birth, my residence, or my volunteer file, to any person or agency outside OPRD. An extensive search has been conducted, and no records exist other than your public records requests. 6. The following policy and training records, without regard to date: Many of these records are publicly available. We’re providing them for you as a courtesy. (a) Statewide Policy 50.010.02, Violence-Free Workplace, in both the version effective 11/1/2024 and the superseded version effective 02/01/2019. 50-010-02.pdf – copy attached. (b) Statewide Policy 50.010.04, Workplace Effects of Domestic Violence, Sexual Assault, and Stalking, and the accompanying online Toolkit referenced in 50.010.02(4). 50-010-04.pdf – copy attached Employee training.docx (toolkit) – copy attached. (c) The DAS-CHRO Violence and Weapons Free Workplace training required by 50.010.02(10), including all slides, scripts, scenarios, handouts, and completion records. Training media will need to be converted into a readable format. Completion records for the training would be extensive since this is a yearly statewide training requirement. Date parameters are needed for these records. Fee estimate for this process is below: Staff Time Estimated Staff Hours Rate Total Clerical (administrative, office specialists, other support staff) 3 $25.00 $75.00 Managerial (Program managers, Public Information Officers) 1 $40.00 $40.00 Total Fee Estimate: $115.00 (d) Any agency safety plan, procedure, guidance, form, checklist, or assessment instrument used to carry out 50.010.02(4) or 50.010.02(6), including any used to assess reported incidents, and any addressing application of the policy to volunteers or other non-employees within the definition of "others working in an agency." Agencies maintain their own internal safety plans and must be requested directly from each agency. Internally, DAS has numerous safety plans and procedures for many program areas. DAS is assessing a fee for employee time searching and producing any responsive documents. At a minimum, we estimate the costs to be $290. To reduce fees, we suggest you consider narrowing your request by being more specific about what type of plan you are seeking. DAS has not issued any of the above other than our publicly available policies. (f) Any record identifying the agency directors, human resource sections, or safety sections designated to administer 50.010.02 at the Oregon Parks and Recreation Department, and any delegation of that authority. DAS does not possess any information nor is it the custodian of records about how the OPRD administers the DAS policy and how they delegate authority. 7. All records reflecting the number of behavioral threat assessments, workplace violence assessments, or threat management processes initiated by executive branch agencies since January 1, 2023, in whatever aggregate or summary form DAS maintains. DAS does not collect information and/or maintain this information. If any responsive record is withheld, please identify it by date, author, recipient, and subject, and cite the specific exemption claimed. If DAS is not the custodian of a particular record, please identify the custodian rather than closing the request. ORS 192.311(2)(b) defines the custodian to include a public body that owns or retains a record, and DAS is the custodian of its own correspondence. No records are being withheld. I request a fee waiver. Disclosure of these records is in the public interest. The question of whether an authority created to prevent violence has been used against a volunteer for documenting his own treatment is a matter of public concern, and the resulting records bear on the conduct of two state agencies toward a member of the public. After careful review, we have determined that your request does not meet the statutory criteria for a waiver or reduction. The request does not demonstrate that disclosure would primarily benefit the general public. Therefore, we must deny the fee-waiver request. We have included fee estimates for the cost of searching and producing any responsive records for your requests #6 (c) and (d). Keep in mind that public bodies may charge for search time even if they fail to locate any records responsive to the request or even if the records located are subsequently determined to be exempt from disclosure. The total cost to fulfill your records request is $450.00 The standard fee structure outlined in statewide policy 107-001-030, the fee for these records is determined in accordance with ORS 192.324(4)(a) and ORS 192.324(4)(b).
