To whom it concerns:
I submitted a tort claim notice by email on September 3, 2026, which your office acknowledged and opened as claim P195403. Today I submitted the same notice through your online form, which opened claim P195427.
My intent was not to submit a duplicate entry. It was to ensure that no future argument can claim the notice was not submitted within the statutory requirements. The two filings are one claim: P195403 is the original, and P195427 is the same notice entered through the Department's own form. Please consolidate them however your office prefers.
